PP-405: The New UCLA Hair Loss Drug, Explained

If you’ve spent any time in hair loss forums or on Reddit this year, you’ve probably seen the name PP-405 floating around. It’s being called everything from “the first real hair loss breakthrough in decades” to “the next Ozempic, but for bald spots.” Neither label is quite right yet, but the science underneath it is genuinely interesting, and it’s worth understanding before the hype gets ahead of the data.
What Is PP-405?
PP-405 is an investigational topical treatment developed by Pelage Pharmaceuticals, a clinical-stage regenerative medicine company built on hair follicle stem cell research out of UCLA. Instead of slowing hair loss the way current treatments do, PP-405 is designed to wake up dormant hair follicle stem cells: the ones that are still present in a balding scalp but have essentially gone to sleep.
That distinction matters. Most existing options manage hair loss. PP-405 is being developed to reverse it by triggering regrowth from follicles that currently produce nothing.
Who’s Behind It, And Who’s Actually Paying For It
PP-405 traces back to research from UCLA professors Michael Jung, Heather Christofk, and William Lowry, who found that shifting the metabolism of hair follicle stem cells could switch them from dormant to active. Pelage Pharmaceuticals licensed that intellectual property from UCLA in 2018 to develop it into an actual drug.
On the funding side, Pelage’s lead investor is GV (Google Ventures), which led the company’s Series A and Series A-1 rounds. In October 2025, Pelage closed a $120 million Series B financing co-led by GV and ARCH Venture Partners, with additional backing from Main Street Advisors, Visionary Ventures, and YK Bioventures. That’s a serious amount of capital and a serious investor syndicate for a company still in clinical trials: a genuine signal of confidence, even if it isn’t proof of a finished product.
What the Phase 2a Trial Actually Showed
In June 2025, Pelage released results from its Phase 2a trial. Here’s what was actually reported, not the headline version:
- 78 men and women with androgenetic alopecia were enrolled, applying PP-405 or a placebo to the scalp once daily for 4 weeks, then followed for 12 weeks total.
- The trial met its safety endpoint: PP-405 was well tolerated, with no systemic absorption detected in the blood.
- At week 8, 31% of men with a higher degree of hair loss showed a greater than 20% increase in hair density, compared with 0% of the placebo group.
- Notably, some new hair growth appeared in follicles where there had previously been none, which is the regenerative effect the therapy is designed to produce, not just thicker existing hair.
That’s a real, statistically significant early signal. It’s also a small trial, reported via company press release rather than a peer-reviewed publication, and the 31% figure describes one subgroup (men with more advanced hair loss) rather than the full study population. Encouraging is the right word. Proven is not — yet.
How PP-405 Would Differ From What’s Available Today
Current hair loss treatments (topical and oral medications, low-level light therapy, Hair Restoration Injections, and hair transplant procedures) largely work by addressing hormones, blood flow, or by physically relocating healthy follicles. PP-405 is aimed at a different target entirely: the metabolic switch inside the stem cell itself. If it works as intended in larger trials, it wouldn’t replace those tools so much as add a new mechanism alongside them, particularly for patients whose follicles have gone fully dormant rather than just thinning.
Elijah’s Hot Take: Cautiously Excited, Genuinely Skeptical
I’ll be honest: I want this to work. A topical that reactivates dormant follicles instead of just slowing the clock would change what we can offer patients who’ve been told there’s nothing left to work with. The science is credible, the team is credible, and the money behind it is credible. That’s not nothing in an industry that’s full of miracle-serum claims with zero data behind them.
But I’ve also been doing this long enough to know that a 78-person Phase 2a trial is early. Very early. We’re talking about a favorable subgroup result, no published peer-reviewed data yet, and a Phase 3 trial that hasn’t even started as of this writing. Hair loss research has a long history of promising early-stage compounds that quietly stalled out or underdelivered once tested on a broader population. I’m rooting for PP-405. I’m not betting a treatment plan on it, and I’d encourage you not to either, not yet.
When Could PP-405 Actually Be Available?
Pelage has said it expects to begin Phase 3 trials in 2026. Phase 3 trials typically run one to several years, followed by FDA review before a drug can be legally marketed and sold. Realistically, that puts any potential approval years away, not months, and Phase 3 could still turn up different results than Phase 2a did. There’s no confirmed timeline, and no guarantee of approval at all.

Is PP-405 the same as minoxidil or finasteride?
No. Minoxidil and finasteride work on blood flow and hormones, respectively. PP-405 targets the stem cells inside the follicle directly: a different mechanism entirely.
